DVA Gold Card and Medicare Levy Exemption Explained

Published

Article at a glance

  • The levy: 2% of taxable income, with a surcharge of 1 to 1.5% for higher earners without private hospital cover.
  • Gold Card counts: the ATO treats it as equivalent to private hospital cover.
  • Full or partial: full exemption for a whole year held, pro-rata for part of a year.
  • Not automatic: you claim it on your tax return with the right certificate.

A DVA Gold Card does more than fund your healthcare. Because the ATO treats it as equivalent to private hospital cover, it may also exempt you from the Medicare Levy.

This guide explains the levy and surcharge, how the exemption works, who is eligible, and how to claim it. For full detail on the card itself, see our DVA Gold Card guide. This is general information, not tax advice, so confirm your position with the ATO or your tax adviser.

What is the Medicare Levy and the Medicare Levy Surcharge?

The Medicare Levy is a tax of 2% of your taxable income that helps fund Australia’s public healthcare system, applied automatically when you lodge your return. Higher earners without private hospital cover may also pay the Medicare Levy Surcharge of 1 to 1.5%. If you already have alternative coverage such as a DVA Gold Card, you may qualify for a full or partial exemption, so you are not penalised for cover you do not need.

How the Gold Card Medicare Levy Exemption works

The ATO recognises that holding a Gold Card means you already have full health coverage, so you may qualify for a full or partial exemption even without private health insurance. If you held the card for the whole financial year you may be eligible for a full exemption. If you held it for part of the year, you can usually claim a pro-rata exemption for those months. The exemption is not automatic: you claim it under Category 1 on your tax return and provide evidence such as a DVA Medicare Levy Exemption certificate.

Am I eligible for the exemption?

For most Gold Card holders the answer is yes, but the level depends on your family situation under the ATO’s conditions for a medical exemption from the Medicare Levy.

  • Full exemption usually applies if you held a Gold Card for the full year and had no dependants, or your dependants were themselves paying the levy or also exempt, supported by a family agreement where needed.
  • Half exemption usually applies if you held the card for part of the year, or had a dependant who was not exempt and did not pay the levy, or share care of a child.
  • Shared exemption applies where you and your spouse both meet the conditions and share a dependent child, with one claiming a full and the other a half exemption under a family agreement.

ADF Medicare Levy Exemption and veterans

Current serving ADF members are entitled to full medical treatment, which the ATO treats as equivalent to private hospital cover, so they qualify for an exemption with the level depending on family circumstances. For former members, the exemption most often flows through the Gold Card. If you do not hold one, you may still qualify for a reduced levy as a low-income earner, which the ATO calculates when you lodge.

How to claim the exemption

  1. Get a Medicare Levy Exemption Certificate from DVA or Services Australia, or use your payment summary for a partial year.
  2. Complete a family agreement if your situation involves a spouse or shared dependants, and keep records of shared-care days.
  3. Indicate on your tax return whether you are claiming for the full year or part of the year.
  4. Keep your documents, as the ATO may request supporting evidence.
